Job summary

Illuminus is seeking a Hospice Patient Care Coordinator (PCC) to provide clinical leadership and mentoring for a compassionate team. You will supervise RNs, LPNs, and Hospice Aids, triage patient care, and participate in IDT meetings to plan care for each patient while supporting admissions and regulatory compliance.

Qualifications include a WI RN license, 4–5 years in hospice, and a valid WI driver’s license.

Qualifications

• Current State of Wisconsin Registered Nurse license.
• Four to five years of Hospice experience; four year degree preferred.
• Valid WI Driver’s license.

Responsibilities

• Provide clinical leadership, guidance, and mentoring for Hospice and staff.
• Supervise and direct care of RNs, LPNs, and Hospice Aids.
• Support/triage patient care visits and clinical questions as RN Case Manager when needed.
• Lead interdisciplinary team meetings to review and revise care plans.
• Assist with Hospice region goals and ensure implementation as scheduled.
• Support intake and referral processes for timely admissions.
• Ensure compliance with local, state, and federal codes; manage on-call services.
