Job summary

Ambercare Hospice in Espanola, NM is seeking a compassionate Registered Nurse Admissions to join our newly restructured team, dedicated to delivering exceptional care with a collaborative spirit.

You will perform pre-admission evaluations, educate patients and families about services, obtain informed consent, and provide an initial nursing assessment and plan of care focused on comfort and quality of life, within the Addus HomeCare family.

Qualifications

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR Certification
• 1 year of Hospice experience, preferred
• Must possess the ability to make independent decisions when circumstances warrant
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public
• Must be knowledgeable of quality assessment and assurance procedures

Responsibilities

• Perform pre-admission evaluations for the appropriateness of services
• Initiates the admission process by educating and informing patients/families of its services
• Obtains informed consent
• Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life
