Hospice & Community Care is seeking an RN for Admissions and Care Transitions in a hospital-based, PRN role covering Lancaster, York and surrounding counties. Orientation is 5 weeks full time, 8am–5pm. 2 shifts per month, 1 weekend per quarter, 1 holiday every 2 years.

Qualifications

• Graduate of an accredited school of registered nursing.
• Maintains current Pennsylvania license.
• One year of nursing experience as a registered nurse within last four years required.
• Hospice experience preferred.

Responsibilities

• Provides accurate and timely information regarding hospice services to anyone inquiring about Hospice care.
• Completes admission visits for routine, general inpatient (GIP) and Hospice Response Team (HRT) patients.
• Performs RN comprehensive assessments and focused skilled nursing visits on GIP/HRT per discipline frequencies.
• Follows up on hospice waiting patients to assure a smooth hospital discharge.
• Documents in the hospital EHR and hospice EMR accurately at the time of service; supports prognosis, level of care, and plan changes.
• Obtains patient data including initial needs assessment, medical history, insurance information, and available resources.
• Communicates assessment information and proposed plan to attending physician and care team.
• Collaborates with facility staff to develop discharge plans.
• Maintains ongoing communication with staff, physicians, patients, and families.
• Assists with discharge planning and hospice enrollment when appropriate.
• Coordinates outreach to referral sources and facilitates referrals.
• Assesses and responds to pain and symptoms of suffering.
• Demonstrates knowledge of palliative care protocols for symptom management.
• Provides direct nursing care as prescribed by physician orders.
• Works independently across care settings as required.
