Graham Healthcare Group is seeking a Hospice RN to join our Home Health team in Youngstown, PA. This full-time role covers Latrobe and surrounding areas with an 8-hour shift and a 40-hour workweek.

We offer a generous bonus potential of up to $15,000, including a $10,000 first-year success bonus and a $5,000 sign-on. Benefits include extensive paid time off, 401(k) contributions, and wellness programs.

Qualifications

• RN license in the state you work.
• Associate degree or higher from an accredited School of Nursing.
• Two years of RN experience, hospice experience preferred.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Skills needed to self‑manage time and schedule.
• Experience with tablets, mobile phones and EMR software.

Responsibilities

• Provide management and delivery of patient care plans.
• Collaborate with AHN Healthcare@Home team to ensure quality hospice care.
• Participate in schedule planning and patient and family education.
