Job summary

Baystate Health is seeking a Hospice Registered Nurse to deliver patient-centered end-of-life care in home and community settings across western Massachusetts, working with physicians, social workers, and the hospice team to relieve suffering and support families. Schedule is full-time days, 8:00 a.m.

to 4:30 p.m., with 40 hours/week and every 6th weekend. A sign-on bonus up to $3,000 is available with 1 year of RN experience; compensation ranges per hour reflect base salaries shown, and

Qualifications

• Associates Degree in Nursing required.
• Active RN license in Massachusetts.
• BLS certification required.
• Experience in hospice or end-of-life care preferred.

Responsibilities

• Provide compassionate, patient-centered nursing care to individuals with advanced illness, focusing on comfort, dignity, and quality of life.
• Assess patients’ physical, emotional, and psychosocial needs and develop individualized plans of care in collaboration with the interdisciplinary team.
• Monitor and manage symptoms such as pain, dyspnea, nausea, anxiety, and other end-of-life concerns using evidence-based nursing interventions.
• Administer medications and treatments safely while educating patients, families, and caregivers on medication use, symptom management, and changes in condition.
• Communicate regularly with physicians, nurse practitioners, social workers, chaplains, aides, and other members of the hospice care team to coordinate services.
• Provide education, guidance, and emotional support to patients and families throughout the hospice experience.
• Respond to changes in patient status and provide timely clinical interventions, including coordination of urgent and after-hours hospice needs.
• Accurately document assessments, interventions, patient responses, and care coordination in accordance with standards.

Our Hospice Team provides compassionate, patient-centered end-of-life care to patients and their families in the home and community setting, working closely with physicians, advanced practice providers, interdisciplinary care teams, patients, and families to deliver expert symptom management, emotional support, and high-quality care focused on comfort, dignity, and quality of life. The Registered Nurse role offers the opportunity to provide meaningful, holistic care to patients and families during the end-of-life journey while developing specialized skills in hospice nursing, coordinating care with an interdisciplinary team, and making a lasting difference in the lives of patients and those who support them.

The Registered Nurse Will:
