Base pay range

$45.00/hr - $75.00/hr

Anarcare Home Health and Hospice has a team of Doctors, Nurses, Counselors, Social Workers, Chaplains, and Volunteers that provide care for patients and support to their families. We deliver care at the times our clients need it most.

Duties and Responsibilities

This position involves communicating with staff, physicians, referral sources, and prospective clients regarding hospice services and admission criteria. It includes conducting initial physical and emotional assessments of patients and families upon admission, reviewing the hospice concept, and developing a detailed, written Plan of Care (POC) in collaboration with the interdisciplinary team (IDT), patient, family, and attending physician.

Qualifications

• Graduate of an accredited nursing school with a current Texas Registered Nurse license.
• Current Basic Life Support certification.
• Excellent organizational skills for handling multiple tasks.
• Effective interpersonal and communication skills, both oral and written.
• Ability to accommodate multi-faith beliefs and backgrounds.
• Ability to work as part of an interdisciplinary team.
• Ability to travel to multiple work locations during the day.
• Reliable, insured vehicle and valid Texas Driver’s license.
