Agrace in Baraboo, WI is seeking a Registered Nurse for the Alt Shift (4pm-12:30am, 28 hours/week, 0.7 FTE). You will provide nursing care, conduct assessments, and coordinate with the care team to support hospice patients and families.
Responsibilities include patient/family assessments, care plan updates, pain management, education, and collaboration with the interdisciplinary team. This on-site role includes on-call rotation, occasional weekends and holidays.

The Registered Nurse Alt Shift (RN Alt Shift) is responsible for patient and family satisfaction through the provision of nursing care and case management by conducting assessments, triaging patient needs, creating and updating the plan of care, providing interventions, communicating, and documenting. The RN Alt Shift provides coverage for care groups across the homecare teams when a need arises (vacancies, FMLA, high census). The RN Alt Shift provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations, and policies. As such, percentages of time listed for each essential function may vary or increase based on needs.
