Qualifications

• Active Minnesota RN license.
• Hospice experience preferred, or strong clinical background in ICU, oncology, palliative care, home health, or other complex care settings.
• Excellent assessment skills with admissions experience and clinical judgment.
• Strong communication and relationship-building with patients, families, and interdisciplinary teams.
• Willingness to travel throughout our service area.

Responsibilities

• Conduct hospice admissions, eligibility evaluations, patient education, informed consents, and initial care planning.
• Provide case management support, including symptom management, care coordination, and clinical oversight as needed.
• Complete routine, PRN, and urgent patient visits to ensure continuity of care.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide holistic, patient-centered care.
• Educate patients and families throughout the hospice journey with compassion and confidence.
• Maintain timely, accurate EMR documentation while ensuring regulatory compliance.
