Job summary

VIA Health Partners in Huntersville, NC is seeking an experienced Registered Nurse to coordinate and deliver patient and caregiver care in a hospice setting. The role emphasizes education, interdisciplinary collaboration, and adherence to infection control and documentation standards.

You will assess patients, develop and update the plan of care, support independence, and participate in IDG conferences. A current RN license and CPR certification are required; CHPN is preferred.

Qualifications

• Diploma or associate degree in nursing required; Bachelor's preferred.
• Current, unrestricted RN license in North Carolina (NLC acceptable).
• CPR certification required; CHPN preferred.
• Minimum 1 year in inpatient units; 2 years for other nursing positions; hospice/palliative care experience preferred.

Keeps supervisor apprised of any complex patient/caregiver issues.Applies principles of infection control. Ensures all areas of facility are clean and appropriately stocked with supplies. Keeps the family room clean throughout the day. Ensures proper chemicals are used for cleaning different surfaces. Ensures protective equipment is used when working with chemicals, i.e., gloves, glasses.Minimum QualificationsA Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.Current CPR required; CHPN preferred.Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.Computer proficiency is required.
