Job summary

PruittHealth is seeking a Hospice Registered Nurse Case Manager (RN) to join our hospice services team. This role involves delivering compassionate, high-quality hospice care in patients' homes or care facilities, with a focus on evaluation, care planning, and documentation.

• Compassionate, patient-focused hospice care delivery.
• Ability to complete evaluations, assessments and treatments per care plan.
