HopeHealth Hospice & Palliative Care is seeking a Registered Nurse (RN) Case Manager to oversee patient care at home and in facilities across Massachusetts. The role emphasizes advocacy, coordination, and timely documentation, with a 40-hour work week (8am-4:30pm) and rotating weekends.

The RN will assess patients, develop and adjust care plans with physicians, and educate patients and families. MA/RI licensure is preferred; BSN is desirable, with 1–3 years of clinical experience.

Qualifications

• RN license required; MA/RI preferred per state licensure.
• BSN preferred; clinical nursing experience in home health or hospice.
• Strong assessment, care planning, and patient/family communication.

About thisHospice Registered Nurse (RN) Position

HopeHealth Hospice & Palliative Care is seeking a Registered Nurse (RN) to care for hospice patients in patient homes and facilities in Massachusetts, primarily in the Milton/Dedham/Westwood or Norwood area.

As a Registered Nurse (RN) Case Manager you will advocate, negotiate, and lead the clinical oversight of the patient. The successful Registered Nurse (RN) is passionate about providing the best possible care at end-of-life and acts with a sense of urgency and a disciplined approach for the accuracy and quality of the care delivered.

This Registered Nurse (RN) position is a full-time, 40-hour per week position working 8am-4:30pm hours. There is a rotating weekend commitment of every 4th weekend.

What will a Hospice Registered Nurse (RN) Case Manager do?

• Conducts assessments for home and nursing home patients
• Conducts initial and ongoing assessment of patients
• Collaborates with physician to manage patient's plan of care
• Evaluates plan of care on a regular basis, and as necessary, to meet the changing needs of the patients
• Provides ongoing training and education to patients and family members
• Initiates appropriate preventive and rehabilitative nursing procedures, making necessary referrals to community services
• Prepares point of care clinical documentation that demonstrates progress toward established goals
• Coordinates services, informing the physician, management, and other staff of changes in the patient's condition and needs.
• Determines scope and frequency of services needed based on acuity and patient needs
• Completes, maintains and submits accurate, relevant and timely documentation regarding patient's condition and care given
