Acts Hospice, part of Acts Retirement-Life Communities, is seeking a Registered Nurse (RN) in Wilmington, Delaware to coordinate hospice plans of care as an active member of the Interdisciplinary Team (IDT). The RN ensures compassionate, professional care aligned with physician orders and the IDT Plan of Care, while advocating for patients and families.

Qualifications

Current State Registered Nurse (RN) license.
Minimum of 2-3 years experience in hospice or related clinical venue.
Current or eligible for CPR certification.
Hospice and palliative nursing certification within two years is encouraged.

Responsibilities

Coordinate the implementation of each hospice plan of care as part of the IDT.
Advocate and educate for improved care of terminally ill patients and families.
Ensure orders and care plans are followed according to physician directives.

Acts Hospice is currently seeking qualified candidates for our Registered Nurse (RN) position.

The Hospice RN is a member of the Interdisciplinary Team (IDT) and is responsible to coordinate the implementation of each hospice plan of care. The Hospice RN ensures that the needs of the hospice participant and family are professionally and compassionately met according to the attending physician’s orders and the IDT Plan of Care. The Hospice RN also serves as an advocate and educator for improved care of all people who are terminally ill.
