Job summary

Three Oaks Hospice, Inc. is seeking a dedicated registered nurse to administer skilled nursing care in patients' homes and coordinate with the interdisciplinary team to ensure seamless, compassionate end-of-life care.

You will work closely with patients and families to implement care plans and monitor progress. The role requires a current RN license, CPR certification, and at least two years of clinical nursing experience; hospice or home health exposure is preferred.

Qualifications

• Graduate of an accredit ed Associate or Bachelors degree in Nursing.
• Current state license as an RN and current state drivers license.
• Two years experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patient needs and ADLs.
• Implement the individualized POC and revisions to the plan as necessary.
• Educate patient and family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs.
• Provide training to other staff as needed.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Coordinate all patient/family services with the IDT.

Position Summary

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence coordinate care with the interdisciplinary team patientfamily and referring agency; and assumes the responsibility for coordination of care

Essential Duties

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical functional psychosocial and environmental needs and ADLs ie risk for grief culturalspiritual verbal non verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Consulting with and educating the patientfamily regarding disease process self care techniques end of life care nutrition and dietary needs
• Providing training to other staff as needed
• Initiating appropriate preventive and rehabilitative nursing procedures
• Preparing clinical and progress notes that demonstrate progress toward established goals
• Coordinating all patientfamily services and prioritization of needs with the members of the IDT
• Use of case management approach and referring to other services as needed
• Informing physician and other personnel of changes in the patients needs and outcomes of intervention while evaluating patientfamily response to care
• Determining scope and frequency of services needed based on acuity and patientfamily needs
• Assessing ability of the caregiver to meet the patients immediate needs upon admission and throughout care
• Evaluating own needs for support and using identified systems to meet the need
• Communicating information using current process and technology available to the organization
• Applying specific criteria for admission and re certification to hospice care to establish appropriate levels of care and the patients eligibility
• Perform on call responsibilities and provide on call service to patientsfamilies as assigned
• Other duties as assigned
