Job summary

Three Oaks Hospice, Inc. seeks a dedicated registered nurse to deliver skilled nursing care in patients’ homes. You will coordinate with the interdisciplinary team, educate families, and adapt plans of care as disease progression requires. Strong computer skills and CPR certification are essential.

Qualifications

• Graduate of an accredited Associate or Bachelor's degree in Nursing.
• Current state license as an RN
• Two years experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient computer skills; Microsoft Office experience preferred

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients and families.
• Develop and update the individualized plan of care (POC) and revise as needed.
• Educate patients and families about disease, self-care, end-of-life care, nutrition and dietary needs.
• Provide training to staff as needed and share nursing expertise.
• Initiate preventive and rehabilitative nursing procedures and document progress.
• Prepare progress notes showing progress toward goals and coordinate services with the IDT.
• Inform physicians of changes in patient needs and outcomes and adjust care accordingly.
• Determine scope and frequency of services based on acuity and needs; assess caregiver capacity.
• Manage on-call responsibilities and provide on-call nursing support.
• Other duties as assigned by the organization.
