Job summary

Three Oaks Hospice in the Austin metropolitan area is seeking a dedicated registered nurse to provide skilled nursing care in patients' homes or residence. You will coordinate with the interdisciplinary team and families to deliver high-quality hospice services while maintaining a compassionate approach.

The role requires an RN license, CPR certification, and hospice/home health experience is preferred. On-call duties and ongoing patient assessment are essential responsibilities in this position.

Qualifications

  • Graduate of an accredited Associate or bachelor’s degree in Nursing.
  • Current state license as an RN and a valid state driver’s license.
  • Two years experience as an RN in a clinical care setting; hospice/home health preferred.
  • Current CPR certification.
  • Proficient computer skills; Microsoft Office experience preferred.
  • Strong interpersonal, communication, and organizational skills; ethic and compliance oriented.

Responsibilities

  • Perform initial and ongoing comprehensive assessments of patient needs and ADLs.
  • Implement the individualized Plan of Care and revise as necessary.
  • Educate patients and families about disease process, self-care, and end-of-life care.
  • Provide training to other staff as needed.
  • Initiate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes showing goal progress.
  • Coordinate services with the IDT and manage priorities.
  • Utilize case management and refer to other services as needed.
  • Inform physician of changes in patient needs and outcomes.
  • Determine scope and frequency of services based on acuity.
  • Assess caregiver capacity and provide support resources.
  • Communicate information using current processes and technology.
  • Participate in admission/re-certification criteria for hospice care.
  • Perform on-call responsibilities and provide on-call service as assigned.
  • Other duties as assigned.

Position Summary

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence coordinate care with the interdisciplinary team patientfamily and referring agency; and assumes the responsibility for coordination of care

Essential Duties
  • Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical functional psychosocial and environmental needs and ADLs ie risk for grief culturalspiritual verbal non verbal
  • Implement the individualized POC and recommend revisions to the plan as necessary
  • Consulting with and educating the patientfamily regarding disease process self care techniques end of life care nutrition and dietary needs
  • Providing training to other staff as needed
  • Initiating appropriate preventive and rehabilitative nursing procedures
  • Preparing clinical and progress notes that demonstrate progress toward established goals
  • Coordinating all patientfamily services and prioritization of needs with the members of the IDT
  • Use of case management approach and referring to other services as needed
  • Informing physician and other personnel of changes in the patients needs and outcomes of intervention while evaluating patientfamily response to care
  • Determining scope and frequency of services needed based on acuity and patientfamily needs
  • Assessing ability of the caregiver to meet the patients immediate needs upon admission and throughout care
  • Evaluating own needs for support and using identified systems to meet the need
  • Communicating information using current process and technology available to the organization
  • Applying specific criteria for admission and re certification to hospice care to establish appropriate levels of care and the patients eligibility
  • Perform on call responsibilities and provide on call service to patientsfamilies as assigned
  • Other duties as assigned
