Suncrest Hospice is seeking an Registered Nurse (RN) to provide in-home and facility-based care across the service area. You will join an interdisciplinary team to deliver compassionate hospice services and perform psych-social assessments, coordinating with social workers and other professionals.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
