Job summary

Mission Healthcare in California is seeking skilled RNs for PRN home health shifts in Contra Costa & Alameda. You will assess patients, develop care plans, and coordinate discharge planning with families to ensure continuity of care.

Applicants need a valid RN license, BLS/CPR, and 1+ year acute care experience. A driving record and nursing degree are required; home health experience is preferred.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS) with hands-on CPR certification.
• Valid driver's license and auto insurance.
• Associate’s or Bachelor’s degree in Nursing; 1 year home health experience preferred.

Responsibilities

• Conduct thorough assessments of patients and their families to determine home care needs.
• Provide comprehensive physical assessments and document the history of illnesses.
• Develop and update individualized care plans with active participation from patients and their families.
• Identify discharge planning needs and ensure they are addressed before patient discharge.
• Offer counseling to patients and families to address nursing and related health needs.

Pay range (depending on experience):

$47.00-$70.00 DOE

PRN, 10am-8:30pm
