Job summary

Mission Healthcare, the leading home health and hospice provider in the western United States, seeks an experienced RN for a hybrid schedule serving the San Gabriel Valley, CA area. You will oversee plan of care, coordinate with physicians, and guide patients and families through treatment and at-home care.

The role requires an active RN license and 1+ year of acute care plus 1+ year in home health, with a compensation range of $60.00–$62.00 hourly, reflecting experience and location.

Qualifications

• Graduate from an accredited nursing program.
• Current state RN license required.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Minimum of 1 year of home care experience, hospice preferred.

Responsibilities

• Initiates the plan of care and revises as patient status changes.
• Uses health data to determine nursing diagnoses.
• Develops care plans with goals based on diagnoses and includes patient/family.
• Provides health care instructions per assessment and plan.
• Assists with activities of daily living and supports patient self-sufficiency.
• Acts as Case Manager when assigned by Clinical Manager to coordinate caseload.
• Communicates with physicians regarding patient needs and changes in condition.
• Teaches patient and family self-care, including medications and diet; coordinates with interdisciplinary team.

Pay range (depending on experience): Full Time $60.00-$62.00 hourly

Schedule/Shift: Monday-Friday + Weekends On Call, Rotation, 12:0pm to 10:30pm

Territory/Location: San Gabriel Valley, CA

Responsibilities:

• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
• Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilize opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group

Qualifications:

• Graduate from an accredited school of nursing. Bachelor’s degree preferred.
• Current state license as a Registered Nurse.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Minimum of 1 year of recent experience in home care, preferably in hospice.
• Valid driver’s license and reliable, insured transportation.
