Job summary

Northeast Georgia Health System is seeking a Registered Nurse for hospice care across multiple settings, including home and facility-based care. You will work with an interdisciplinary team to deliver high-quality nursing care, participate in on-call coverage, and travel within the service area to meet patient needs.

Ideal candidates have GA RN licensure, BLS, and at least 1 year of clinical nursing experience; CHPN is preferred for enhanced hospice expertise.

Qualifications

• GA RN licensure and BLS certification required.
• Associate degree in nursing minimum.
• CHPN preferred for hospice care.
• One year of clinical nursing experience required.

Responsibilities

• Admit patients to the hospice program and assess suitability.
• Complete admission visit and document findings promptly.
• Deliver nursing care per ANA standards and care plans.
• Supervise hospice aide and LPN per regulations.
• Provide on-call coverage after normal hours as scheduled.
• Educate patients and caregivers; ensure safe environment.
• Document care and coordinating paperwork for payors.
• Communicate with IDT to optimize patient plans.
