Job summary

Interim HealthCare Home Health and Hospice in Michigan is seeking a Hospice RN Case Manager to provide compassionate, autonomous home-based nursing care and meaningful patient relationships.

You will work with physician teams, oversee CNAs, educate families, and coordinate care plans and discharges. Requirements include an active Michigan RN license, CPR, and at least 1 year of hospice experience. The package includes salary and benefits.

Qualifications

• Must hold an active RN license in Michigan.
• CPR certification required.
• Minimum of 1 year of clinical experience as an RN; hospice experience preferred.

Responsibilities

• Provide home-based care to patients with terminal illness.
• Collaborate with physicians and oversee CNAs delivering patient care.
• Perform assessments, document changes, and report to physicians.
• Educate patients on care plans, medications, diet, and exercises.
• Coach families on care plans, medications, and home safety.

Hospice NurseResponsibilities:

• Provide home-based care to patients with terminal illness
• Work with a team of physicians and oversee CNAs providing patient care
• Examine patients, do assessments, document and report changes to their physician
• Educate patients on their plan of care, prescribed medication, diet and exercise
• Coach family members on the patient’s plan of care, medication and home safety
• Ensure goals are met and coordinate discharge from services

A few must-haves for Hospice Registered Nurses:

• Registered Nurse (RN) with active, unrestricted license in the State of Michigan – BSN not required
• At least one (1) year of clinical experience as a Registered Nurse (preferably in Hospice)
• Current CPR Certification
• Reliable transportation with a valid state driver's license and current car insurance
