Centric Home Health and Hospice is seeking a dedicated Hospice Registered Nurse (RN) to join our team. The role focuses on patient visits in home settings, coordinating care across disciplines, and leading care plans with a compassionate, patient-centered approach.

Ideal candidates bring strong hospice experience, excellent communication, and the ability to work collaboratively with physicians, caregivers, and a multidisciplinary team. Travel to patient homes is required.

Competenze

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• Current RN license in Oklahoma.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Two years of clinical experience; hospice experience preferred.

Mansioni

• Conduct initial and ongoing comprehensive patient assessments ensuring hospice eligibility criteria is met.
• Develop, implement, and revise individualized plans of care.
• Educate patients and families on disease processes, self-care, nutrition, and end‑of‑life needs.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Provide clinical direction to Hospice Aides and LPNs/LVNs.
• Maintain accurate, timely documentation of care and progress.
• Participate in on‑call rotation and respond to patient needs promptly.
• Follow infection control and safety procedures
• Uphold agency policies and professional standards
