Qualifications

• Graduate of a National League of Nursing accredited school of nursing.
• Current California RN license and CPR certification.
• Minimum of 1 year experience as a professional nurse within the last 3 years or a BSN with current RN license and 2 years in public health/home care preferred.
• Must have a valid driver's license with insured vehicle.
• Self-directed and able to work with minimal supervision.
• Excellent observation, verbal and written communication skills; basic math and nursing skills.

Responsibilities

• Complete initial, comprehensive and ongoing assessments to determine hospice needs.
• Provide professional nursing care per the nursing process and physician plan of care.
• Write, initiate and revise the plan of care with patient and family.
• Administer medications and treatments as prescribed.
• Counsel patient and family in meeting nursing and related needs.
• Act as Case Manager when assigned and coordinate patient care.
• Complete accurate clinical notes and communicate with physicians and care team.
• Coordinate with the interdisciplinary team to ensure continuity of care.
• Participate in on-call duties and ensure necessary equipment and services are available.
