Job summary

Qualifications
- Graduation from an NLN-accredited school of nursing.
- Current California RN license and CPR certification.
- Minimum two years’ experience in public health, home health, or hospice nursing preferred.
Responsibilities
- Complete initial and ongoing patient assessments to determine hospice needs and develop care plans.
- Provide professional nursing care per the nursing process and state Nurse Practice Act.
- Record clinical notes and patient status; communicate changes to physicians and care team.
- Coordinate patient care as Case Manager when assigned; collaborate with interdisciplinary team.
