Job summary

Grace Healthcare Services in Sussex County, New Jersey, seeks a Hospice Registered Nurse (RN) to join our interdisciplinary team. You will assess patients, initiate care plans on admission, and ensure compassionate, continuous support throughout the patient journey.

The role requires an active New Jersey or multi-state RN license, at least two years of acute-care experience, and strong communication with patients and families.

Qualifications

• Valid New Jersey or Multi-State Registered Nurse (RN) license.
• Minimum of two (2) years of nursing experience in acute-care settings such as medical, surgical, or oncology units.
• Strong clinical practice with a focus on customer service, excellent communication abilities, and the capacity to work independently in a fast-paced environment.

Responsibilities

• Deliver skilled nursing care per individualized care plans for terminally ill patients.
• Collaborate with physicians, social workers, chaplains, nursing assistants, and others to develop and adjust care plans.
• Educate and support patients' families and primary caregivers with guidance and emotional support.
• Serve as a liaison between patients, families and healthcare providers to advocate for patient wishes.
• Travel daily to patient locations and facilities as assigned.

Role Overview

As a Registered Nurse (RN) at Grace Healthcare Services, you will be an integral member of our Interdisciplinary Team (IDT), responsible for assessing and addressing the comprehensive needs of patients and their families. Your role involves initiating appropriate interventions upon admission and ensuring a continuous, responsive plan of care throughout the patient's journey.

Key Responsibilities
