Enhabit Home Health & Hospice is seeking skilled professionals to deliver compassionate home-based care across our nationwide network. You will coordinate care, assess patient and family needs, and implement the interdisciplinary plan of care while adhering to physician orders and regulatory requirements.

Qualified candidates are licensed in their state, have completed an approved program, and hold CPR certification.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they practice.
• At least one year of experience as a licensed professional.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Coordinate care and assess patient and family needs.
• Implement the interdisciplinary care plan.
• Ensure delivery of care complies with federal and state regulations.
