Job summary

Phoenix Home Care's Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and families dealing with a terminal diagnosis, collaborating with an interdisciplinary team to develop patient-centered plans of care. The role is based in Topeka, KS and surrounding areas, with days Monday–Friday.

The ideal candidate will bring energy, strong managerial and interpersonal skills, and the ability to guide patients and families through complex healthcare decisions in demanding

Qualifications

• Active Kansas or Compact RN license.
• Valid driver's license and auto liability insurance.
• Hospice experience preferred.

• Provide nursing care, education, and support to patients and families
• Complete initial and ongoing assessments
• Coordinate care with physicians and the hospice interdisciplinary team
• Develop and update patient-centered plans of care
• Provide guidance with healthcare decisions

• Pay Range: $75,000–$90,000 | Full-Time | Monday–Friday, Days

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Our ideal candidate should possess a high level of energy and motivation, coupled with strong managerial, organizational, and interpersonal skills. They must be capable of providing precise and effective guidance and support, even in demanding situations, in order to enhance learning opportunities and foster professional growth.

Requirements
