Ascend Hospice & Palliative Care NJ is seeking an RN Case Manager to support on-call, case management, and visits across our hospice program. The role requires an Associate’s Degree in Nursing and a State Certified RN license, plus willingness to travel and drive.

You will collaborate with the healthcare team to assess patients, plan and adjust care, and ensure continuous, high-quality end-of-life care for patients and families. Competitive salary and benefits offered.

Responsibilities

• Complete an initial patient and family assessment to determine hospice care needs.
• Regularly re-evaluate the patient’s nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team to coordinate continuous patient care.
• Counsel patient and family on end-of-life needs.
• Initiate appropriate preventive and palliative nursing procedures.
• Communicate with the physician about patient needs and changes.
• Supervise the Hospice Aides.

Now Hiring – RN Case Manager

Starting Salary:90,000/Year

TheRN Case Managerwill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re-evaluates the patients nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
5. Counsels the patient and family in meeting end of life needs
6. Initiates appropriate preventative and palliative nursing procedures
7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
8. Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills.
