Job summary

Ascend Hospice is hiring a RN Case Manager in New Jersey. The role focuses on on-call/case management and visits to ensure coverage and effective coordination of hospice care.

Starting salary is $90,000 per year. The position requires an RN license, 2 years of nursing experience (hospice preferred), and willingness to travel with valid driving credentials.

Qualifications

• Must have State Certified RN license.
• 2 years' nursing experience; hospice experience preferred.
• Must have a valid driver’s license and reliable transportation.
• Must be willing to travel.

Responsibilities

• Complete initial patient and family assessments to determine hospice needs.
• Regularly reevaluate patient nursing needs.
• Use data to determine and adjust care plans.
• Collaborate with the healthcare team to coordinate continuous care.
• Counsel patient and family on end-of-life needs.
• Initiate preventative and palliative nursing procedures as appropriate.
• Communicate with physician about patient needs and changes in condition.
• Supervise Hospice Aides.

Starting Salary:90,000/Year

TheRN Case Managerwill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re-evaluates the patients nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
5. Counsels the patient and family in meeting end of life needs
6. Initiates appropriate preventative and palliative nursing procedures
7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
8. Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills.
