Qualifications

  • High School Diploma or GED equivalent required.
  • 1 year of experience as a Hospice Aide or Nursing Assistant is required.
  • State-established Hospice Aide training program that meets the requirements of 42 CFR §484.36.
  • Competency evaluation program
  • Experience in a home health environment is preferred.
  • 50% travel required.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
  • Valid driver’s license and insurance.
  • Reliable transportation to perform job tasks

Responsibilities

  • Verify the Plan of Care prior to each visit and provide care according to RN delegation, physician’s orders, assessment data, and established standards and guidelines
  • Incorporate patient care goals from the Plan of Care into nursing visits and document them in skilled nursing notes
  • Communicate significant findings, problems, and changes in patient condition or environment to the RN Case Manager, physician, clinical supervisor, and other involved personnel
  • Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician
  • Implement patient safety plans using patient, family, and community resources
  • Assist patients in learning appropriate self-care techniques while delivering quality care
  • Suggest revisions to the Plan of Care and notify the RN Case Manager of identified patient care issues
  • Provide services requiring substantial and specialized nursing skills under the direction of a Registered Nurse or physician
  • Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions
  • Document medication regimens and update medication profiles according to established procedures
  • Treat patients and caregivers in the highest and most effective manner
  • Implement actions to prevent avoidable hospitalizations and ER visits
  • Provide positive, supportive communication to physicians, patients, families, visitors, and agency personnel
