Job summary

MEP Health is seeking a compassionate Hospice Nurse to provide after-hours care to patients in Milwaukee North and surrounding areas. You will perform thorough patient assessments, respond to concerns from patients, families, and caregivers, and conduct scheduled or as-needed in-person visits as clinically indicated.

The role requires strong nursing judgement to manage end-of-life symptoms, adherence to orders and protocols, and coordination with an interdisciplinary team to support

• Experience in hospice and/or end-of-life symptom management
• Strong clinical assessment and nursing judgment
• Ability to triage concerns and respond appropriately in urgent situations
• Skilled communication with patients, families/caregivers, and care team
• Proficient in implementing orders, protocols, and care plans
• Ability to coordinate effectively with interdisciplinary providers
