Job summary

Cantex is seeking a Hospice Registered Nurse (RN) to provide skilled nursing care to hospice patients in their homes in Livingston, TX. The RN will assess patients, develop and manage care plans with the IDT, and ensure quality, compliant hospice care.

Qualifications

• RN license required (state-specific)
• Hospice experience preferred
• Home Care Home Base (HCHB) experience preferred
• Strong clinical assessment, communication, and documentation skills

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments for hospice patients
• Develop and update individualized hospice plans of care
• Coordinate patient care with IDT, physicians, patients, and families
• Educate patients and families on disease processes, medications, and end-of-life care
• Ensure compliance with Medicare hospice regulations and agency policies
• Maintain accurate clinical documentation in EHR
• Supervise Hospice Aides and LVN/LPN staff
