Job summary

Cantex is seeking an experienced Hospice Registered Nurse (RN) to provide skilled nursing care to hospice patients in their residences within the Grayson County/Denison/Gainesville territory. The RN will perform comprehensive assessments, develop and manage hospice plans of care, and ensure regulatory compliance while collaborating with the Interdisciplinary Team, physicians, patients, and families.

Qualifications

• Active RN license per state requirements.
• Hospice experience preferred.
• Home Health Base (HCHB) experience preferred.
• Strong clinical assessment and communication skills.
• Ability to work independently and in IDT.

Responsibilities

• Perform initial and ongoing hospice assessments.
• Develop and update individualized hospice plans of care.
• Coordinate patient care with the Interdisciplinary Team (IDT), physicians, patients, and families.
• Educate patients on disease, medications, symptom management, and end-of-life care.
• Determine scope and frequency of nursing services based on patient condition.
• Supervise Hospice Aides and LVN/LPN staff.
• Maintain accurate clinical documentation in the EMR.
• Participate in IDT meetings and case conferences.
• Ensure Medicare hospice documentation compliance.
