Qualifications

Responsibilities

• Provide management and delivery of individualized hospice patient care plans
• Conduct comprehensive assessments and ongoing evaluations of patient needs
• Consistently meet critical deadlines for charting and documentation
• Collaborate with interdisciplinary hospice team members to ensure coordinated care
• Educate patients and families on care plans, symptom management, and available resources
• Self-manage time, schedule, and daily patient visits
• Maintain compliance with all regulatory, clinical, and organizational standards

Key Responsibilities

Schedule & Shift Details

Full‑time weekday schedule
On‑call rotation: Every 3rd week
Occasional local travel to patient homes and facilities

Required Qualifications

• Associate Degree or higher
• Valid Registered Nurse (RN) license in Pennsylvania (PA)
• 2+ years of Hospice experience
• 1+ years of experience in any of the following: Electronic Medical Record (EMR), Other EMR systems, TruChart EHR
• Strong ability to self‑manage time, schedule, and documentation
• Compassionate approach to end‑of‑life care
• Excellent communication and patient advocacy skills
