As a Hospice RN / RN Case Manager, you will:
Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary control and professional management of each patient.
Serve as the primary liaison between the physician, patient/family, and hospice team.
Provide direct patient care to hospice patients and supportive care to their families, managing a reasonable caseload while building trusting and compassionate relationships.
Empower caregivers through education on caring for their loved ones.
Collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to ensure comfort, peace, and dignity for patients.
Assess patient and family needs and assist in developing and implementing the plan of care.
Maintain accurate clinical records for all assigned patients.
Participate in quality assessment and performance improvement initiatives.
About You
Hospice Registered Nurse / RN Case Manager Requirements:
Compassionate, kind, and empathetic toward patients and families.
Comfortable providing care in patients' homes.
Able to demonstrate flexibility and composure during crises.
RN license in the state of employment or eligibility to obtain one.
At least 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology is preferred.
Additional preferred experience includes hospital staff RN, med-surg, PACU, ER, critical care, cardiac, wound care, telemetry, or travel nursing.
Valid driver’s license and auto insurance.
Current CPR certification.
