Job summary

Angels Care Hospice in Arizona is hiring a Registered Nurse (RN) Case Manager to coordinate all aspects of home care. You will perform initial home visits, assess eligibility for home care services, and develop the plan of care with the patient and family.

The role emphasizes professional nursing judgment, collaboration with interdisciplinary teams, and delivering compassionate care in patients’ homes while pursuing a long-term career with a supportive employer.

Qualifications

• Graduate of an accredited diploma, associate, or baccalaureate nursing program.
• Current state RN license.
• Valid driver's license.
• One year RN experience; two preferred.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with auto insurance.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
