Job summary

Heartland Hospice seeks a Hospice Care Consultant to build referral partnerships with hospitals, SNFs, home health agencies, clinics, and physician offices across the United States. You will educate healthcare professionals about hospice options and collaborate with clinical and operations teams to ensure timely, appropriate referrals.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs, or home health professionals.
• Prior hospice, palliative care, or home health experience is a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in Microsoft Office and CRM platforms.
• Sensitivity to the needs of terminally ill patients and families.
• Proven track record of meeting admissions and census targets.

Responsibilities

• Educate and inspire confidence in hospice services to drive referrals.
• Identify new referral opportunities across hospitals, home health agencies, SNFs, clinics, and physician offices.
• Engage with healthcare professionals about patient needs and hospice support.
• Plan and execute strategic marketing initiatives to grow referrals.
