Job summary

Crossroads Hospice & Palliative Care seeks a Quick Response STNA (CNA) to provide urgent care during active end-of-life stages. You will be the steady presence for patients and families, addressing evolving needs and coordinating with an interdisciplinary team.

The role requires Ohio CNA certification, 1 year hospice or home health experience, and a valid driver's license for travel to Green, OH. Competitive benefits and scheduling offered.

Qualifications

• Interpersonal skills to interact with patients, families and caregivers.
• Ability to learn required clinical software and electronic record systems.
• Must have a valid driver’s license and reliable vehicle for travel to Green, OH.

Responsibilities

• Provide urgent support during active stages of dying as part of the Even More Care program.
• Serve as a steady presence for families and patients, addressing evolving physical or emotional needs.
• Travel to Green, OH office as required and collaborate with interdisciplinary team.
