Job summary

Choices Healthcare Medical Services and Palliative Medicine Team is seeking a Physician to provide hospice and palliative care consultations and visits across home, clinic, inpatient, and hospital settings. You will help develop medical policies and supervise students and residents while complying with state and national regulations.

The role requires PA medical licensure, board eligibility in Hospice and Palliative Medicine, and strong communication with interdisciplinary teams.

Qualifications

• Licensed to practice medicine in Pennsylvania.
• Board eligible and/or certified in Hospice & Palliative Medicine or as Hospice Medical Director.

Responsibilities

• Provide consultations and visits for hospice and palliative care patients.
• Lead clinical teams and coordinate care across home, clinic, inpatient, and hospital settings.
• Develop and implement medical policies, procedures, and protocols.
• Perform on-call coverage including weekends/holidays.
