Job summary

Four Seasons Care in North Carolina is seeking a physician (MD or DO) to provide direct hospice/palliative care across hospital, hospice facility, and home settings. You will collaborate with a multidisciplinary team to implement and evaluate care plans, support patient and family education, and ensure compliance with Medicare Conditions of Participation.

Position requires a medical license in North Carolina, board eligibility or certification in Hospice and Palliative Medicine, and DEA

Responsibilities

• Provide direct client care as attending or consulting physician in hospice/palliative settings.
• Participate in weekly Interdisciplinary Team meetings to review care plans.
• Coordinate patient hospice care with professional standards.
• Contribute to plan of care development and evaluation.
• Support patient and family counseling and education.
• Comply with Medicare Conditions of Participation.
• Evaluate patients for research participation.
• Maintain relationships with community physicians.
• Serve as designee of Hospice Medical Director as needed.
