UnitedHealth Group in Oregon seeks a dedicated Physical Therapist to provide age-appropriate hospice services, including assessment, direct treatment, home exercise programs, family education, and documentation in accordance with hospice standards. You will join an interdisciplinary team, participate in care planning, and discharge summaries, with responsibilities to educate patients and families.

A current Oregon PT license, CPR certification, and valid driver's license are required; salary

Qualificações

• Current Physical Therapy licensure in state of Oregon.
• Current CPR certification or ability to complete within 90 days of hire.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilidades

• Initiates physical therapy evaluation according to physician orders and Plan of Care for symptom control or to enable the patient to maintain activities of daily living and basic functional skills
• Documents in the patient's clinical record each visit and provides report including patient reaction to treatment and any changes in status
• Participates in the Interdisciplinary Group and provides input for the Plan of Care
• Instructs patient, family and Interdisciplinary group on functional limitations and progress toward goals
• Prepares a written discharge summary when services are discontinued and forwards copy to physician
• Instructs patient and family in phases of physical therapy as appropriate
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
