Job summary

OnePoint Patient Care is hiring a full-time Pharmacist in Las Vegas, NV to work in a closed-door hospice pharmacy setting, delivering high-quality pharmaceutical care to hospice patients.

You will evaluate prescriptions, collaborate with a multidisciplinary team, and maintain patient profiles while ensuring compliance with regulatory standards. Eligible candidates possess a pharmacy degree and current licensure; Pharm D is preferred.

Qualifications

• Bachelor's Degree in Pharmacy with current licensure in the state.
• Pharm D preferred and familiarity with CMS/DEA/FDA guidelines.
• Knowledge of drug therapy guidelines and medication safety practices.
• Proficiency in EMR and pharmacy information systems.

Responsibilities

• Receive and evaluate prescriptions for drug appropriateness and dosing.
• Collaborate with physicians and nurses to provide clinical support.
• Maintain patient profiles with medical history and meds.
• Provide clinical recommendations on medication therapy changes.
• Ensure compliance with state/federal regulatory requirements.
• Supervise IV preparation, Narcotics room, and Omnicell systems when applicable.
