You will assist patients with daily activities, document care, and collaborate with nurses to tailor support for each patient. The role emphasizes dignity and respect, requiring a valid Ohio driver’s license and the ability to travel within the service area.

Qualifications

• High school diploma or GED required.
• Active Ohio STNA/CNA certification in good standing preferred.
• One year of healthcare or patient care experience preferred.
• Hospice and Palliative Nursing Assistant (CHPNA) certification preferred.
• Basic computer skills for electronic documentation.

Responsibilities

• Assist patients with personal care activities while preserving comfort, dignity, and independence.
• Support family members by providing education and assistance with daily care needs.
• Observe and report changes in patient condition to the clinical team.
• Collaborate with nurses and other hospice professionals to help deliver individualized patient care.
• Accurately document care and services in the electronic medical record.
• Maintain professional, compassionate communication with patients, families, and team members.
