Ohio's Hospice is seeking a Personal Care Specialist to provide direct patient care in homes and facilities across the Washington Court House area. This full-time role involves 4-10 hour days Sun-Wed, 8am-6pm.

You will document services, assist with personal care, and collaborate with a compassionate team to support patients and their families through end-of-life care. A valid Ohio CNA preferred; driving required.

Qualifications

• High school diploma or GED required.
• CNA certification (OH) preferred.
• Experience in home care or hospice environments encouraged.
• Valid driver’s license and insurance.

Responsibilities

• Assist patients with personal care needs in homes or facilities.
• Document services and care using electronic systems.
• Support families and collaborate with the care team.
• Drive to patient locations and ensure safety.

What You Should Know About the Hospice PCS Role:

• This position will see patients in homes and facilities in the Washington Courthouse areas of coverage
• Full Time, 4-10hr days of Sun-Wed 8a-6p

The Hospice PCS Duties Are:

We provide superior care for patients in families in homes/facilities. Our aides are titled "Personal Care Specialist" because they are valued for their expertise in taking care of the patient's personal needs with dignity, compassion and empathy. You could become a Personal Care Specialist that provides quality nursing assistance to patients, assists the family with the care of the patient and works with a team that relies on your input from the close relationship you develop with the patient.

Qualifications:

• High school diploma or GED;
• Certified Nursing Assistant with the State of Ohio preferred
• one year of experience preferred
• Certification in Hospice & Palliative Medicine (CHPNA) preferred within two years of employment.
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's license with Safe Driving Record.
• State minimum automobile insurance coverage.
