Job summary

Dignity Care Hospice – A Division of CommuniCare Health Services in Blue Ash, OH is seeking a compassionate Patient Navigator / Navigator Liaison to support patients and families through care planning and transitions. The role bridges patients, families, and the healthcare team.

Candidates should be a licensed practical nurse or registered nurse with at least three years of clinical nursing experience, with strong communication and knowledge of advanced care planning; travel to service sites

Qualifications

• LPN or RN with a valid nursing license.
• Minimum of 3 years of clinical nursing experience (home health, hospice, or skilled nursing preferred).
• Strong knowledge of advanced care planning and healthcare regulations.
• Excellent interpersonal and communication skills with the ability to build rapport with patients and families.
• Ability to work both independently and collaboratively with interdisciplinary teams.
• Strong organizational and time-management skills.

Responsibilities

• Conduct comprehensive assessments of patients to evaluate medical, emotional, and social needs.
• Collaborate with interdisciplinary teams (physicians, social workers, therapists, etc.) to develop individualized care plans.
• Assist patients and families with advanced care planning, including goals of care, treatment preferences, and end-of-life decisions.
• Serve as a liaison between patients, families, and providers to facilitate smooth care transitions.
• Educate and support patients and families on available hospice and healthcare resources.
• Monitor patient progress and adjust care recommendations as appropriate.
• Maintain accurate documentation of assessments, care plans, and interactions in compliance with regulations.
• Participate in quality improvement initiatives and help develop best practices in patient navigation.
