Job summary
Life Care Home Health Family is hiring a Hospice Patient Care Assistant to support the interdisciplinary team in providing timely, cohesive hospice services. The role involves maintaining patient records, coordinating care plans, and ensuring smooth communication among staff, patients, and families.
Ideal candidates hold an LPN or MA graduation certificate and have 2+ years of recent acute/sub-acute care experience.
Qualifications
- LPN or MA graduation certificate with license in good standing.
- At least 2 years of recent experience in an acute/sub-acute care setting.
- Ability to perform nursing activities within LPN or CNA scope and to respond to patient/family needs.
Responsibilities
- Open, maintain, and close clinical records per approved format.
- Act as liaison between team members and patients/families.
- Coordinate care plan activities and scheduling with the Director of Patient Care Services.
- Notify team of patient discharge and ensure documentation is timely.
- Arrange transport for location changes and handle related paperwork.
- Order and monitor supplies and durable medical equipment.
