Concho Hearts Hospice in Brenham, TX is seeking a personable Hospice Marketer / Community Liaison to grow our presence across Brenham, Washington County, and surrounding communities. This full-time role focuses on educating healthcare partners, building trusted referral relationships, and helping patients access compassionate hospice care when needed.

Responsibilities

• Build and maintain relationships with physicians, hospitals, skilled nursing facilities, and other referral partners.
• Identify new referral opportunities and develop strategies to increase hospice awareness and admissions.
• Represent Concho Hearts Hospice in Brenham and surrounding communities.
• Educate healthcare professionals about hospice services and eligibility.
• Conduct regular visits and follow-ups with referral sources.
• Develop and manage a marketing schedule and territory plan.
• Collaborate with clinical and leadership teams to ensure positive referral experiences.
• Participate in community events, networking, and educational presentations.
• Track referral activity and marketing progress, maintaining accurate records.
• Aim for growth goals while upholding ethical and compliant marketing practices.
