Job summary

Chapters Health System in Florida Keys & North Florida seeks a Social Worker to provide psychosocial support for patients and families across hospice, palliative, and home health settings. The role emphasizes compassionate care, collaboration with the care team, and thorough documentation.

The ideal candidate will hold a CSWE-accredited MSW, have 1+ year health care experience, and be able to travel to off-site locations while maintaining high ethical and professional standards.

Qualifications

• Master’s degree or higher in Social Work (CSWE accredited).
• Minimum of one year of social work/counseling internship in health care.
• Experience with pediatric and adult hospice/palliative care concepts.
• Familiarity with EMR/EHR systems and documentation best practices.
• Ability to travel to off-site locations and manage caseloads.
• Valid driver’s license and auto insurance per policy.

Responsibilities

• Performs psychosocial assessments and documents findings.
• Empowers patients/families to negotiate resources and plans.
• Collaborates with care teams to ensure quality, continuity of care.
• Maintains timely, complete and accurate documentation.
• Participates in IDG meetings and supports interdisciplinary care.
