Join a respected and compassionate team of caregivers serving the greater Capital Region of upstate New York.

Location: NYs Capital District

170 miles to Boston

Group serves over 4,000 patients annually, with an average daily census of 625 patients. It collaborates with 43 skilled nursing facilities and many assisted living/adult homes across the service area.

Physician Responsibilities:

• Meet routinely with the interdisciplinary hospice team (nurses, social workers, chaplains, volunteers) for care planning tailored to patients and families.
• Serve as a resource and collaborate with community medical providers, including prescribing medications.
• Certify initial and ongoing eligibility for hospice services.
• Determine appropriate level of hospice care.
• May serve as the attending physician for hospice patients.

Physicians visit patients in their homes, nursing homes, and hospitals as needed, and participate in education of medical students, NP and PA students, residents, and Geriatrics Fellows. On-call duties are shared among a group of 5 hospice physicians.

Fellowship in Geriatrics or Hospice & Palliative Care is a plus but not required.
