HOC Navigators in partnership with TriHealth has a rewarding full-time opportunity for Palliative Medicine physicians in Cincinnati, Ohio. The ideal candidate will be BC / BE in Hospice / Palliative Medicine and have a passion for providing care that meets the patient’s desires and goals for treatment and comfort in an inpatient setting.Hospital-based and outpatient opportunities available.

Qualifications

  • Ability to obtain an unrestricted Ohio Medical License and DEA license
  • Fellowship-trained / Board Certified or Eligible in Hospice and Palliative Medicine
  • Communication skills to lead family meetings, goals of care discussions and end-of-life discussions with cultural humility
  • Complete psychosocial and spiritual assessments in collaboration with the interdisciplinary team
  • Manage pain and non-pain symptoms that accompany serious illness
  • Provide end-of-life care with an understanding of hospice eligibility criteria and the ability to appropriately refer patients
  • Provide a supportive team environment through expressed enthusiasm, approachability and respect for all patients and all members of the health care team
  • Ability to work within an interdisciplinary team and provide consultative care through :

Openness to feedback from colleagues

  • Prior experience working with an interdisciplinary team
  • Experience precepting learners on palliative medicine topics

Responsibilities

  • Provide care services to the patient and his / her family to identify, control or alleviate physical and emotional discomfort and other symptoms that often accompany a serious health condition
  • Ability to hold intensive meetings with family members ensuring that delivery of care is coordinated across the health care setting
  • Work closely with other clinicians in a team approach to patient care
  • Be engaging and approachable in a highly visible position that is always responsible for creating a positive impression with patients administration and others that he / she encounters, both in person and on the phone
  • Enjoy teaching opportunities with resident physicians as well as medical students
  • Provide support for all members of Administrative and Management Teams

Throughout our 40+ year history as a non-profit organization, we continue to be on the forefront of advanced illness and end-of-life care both locally and nationally.We are the largest hospice in Cincinnati, with four Inpatient Care Centers located throughout the Cincinnati region for the treatment of uncontrolled symptoms.

In 2020, Hospice of Cincinnati and the multiple advanced illness and grief programs created by Hospice of Cincinnati, joined together to form HOC Navigators– a collection of program offerings designed to help guide patients through advanced illness and end of life. Today HOC Navigators Palliative Care providers serve patients in six hospitals, two clinics and in patient homes throughout Greater Cincinnati.
