Qualifications

• BC/BE in Palliative & Hospice Care
• Fellowship in Hospice & Palliative Care desired
• Experience in hospice-palliative care preferred

Responsibilities

• Provide consultation on patient care and management to patients in hospice and to interdisciplinary team members.
• Assist in evaluating the provision and scope of medical services within the hospice program.
• Maintain compliance with regulatory changes and documentation requirements related to end-of-life care.
• Support and collaborate with on-site complex case management, social workers, and chaplains.
